29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level was not guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder.  The railing shall be provided with a toeboard wherever, beneath the open sides:  a.  On or about August 5, 2016, at Armstrong World Industries, Inc., in the priming and intermediate (PNI) area of the plant, the employer did not provide standard railing to an opening on top of the press machine while maintenance employees were required to perform repairs, exposing employees to a 13 feet 1 inch fall hazard.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  a.  On or about August 5, 2016 at Armstrong World Industries, Inc., the employer did not conduct periodic inspections at least annually on lockout/tagout (LOTO) procedures and requirements.  Employees were not locking out machines and did not have record of periodic inspections for the machines they operated.  Employees are exposed to pinching, crushing, amputations and permanent disabling injuries.

29 CFR 1910.242(b):  Effective chip guarding and personal protective equipment was not used when compressed air was used for cleaning purposes:  a. On or about August 5, 2016, at Armstrong World Industries, Inc., in the priming and intermediate area of the plant, employees were using hoses with compressed air with no chip guarding that had a psi greater than 30 psi to blow dust that contained metal particles from machines and floors.  Employees were not wearing face shields, long pants, or long sleeves as protective clothing, exposing employees to struck by hazards

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a.  On or about August 5, 2016, at Armstrong World Industries, Inc., the employer did not provide safety guards to cover rollers, belts and pulleys while operating the Tenorer machine Jump # 5.  Employees are exposed to pinching, crushing and amputations.   b.  On or about August 5, 2016, at Armstrong World Industries, Inc., the employer did not provide safety guards to in-going nip points on powered rollers and conveyor belt located each side of the Tenorer machine Jump # 2.  Employees are exposed to pinching, crushing and amputations.   c.  On or about August 5, 2016, at Armstrong World Industries, Inc., the employer did not provide safety guards to in-going nip points on powered rollers and conveyor belt on the outgoing side of the sander.  Employees are exposed to pinching, crushing and amputations.    Armstrong World Industries, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1058735, citation number 01, item number 01 and was affirmed as a final order on July 10. 2015, with respect to a workplace located at 1735 N. Washington St., Warren, MS 39183.
