29 CFR 1910.137(c)(2)(xii): The employer shall certify that equipment has been tested in accordance with the requirements of paragraphs (c)(2)(iv), (c)(2)(vii)(D), (c)(2)(viii), (c)(2)(ix), and (c)(2)(xi) of this section. The certification shall identify the equipment that passed the test and the date it was tested and shall be made available upon request to the Assistant Secretary for Occupational Safety and Health and to employees or their authorized representatives:    On or about 03 August 2016, the employer did not ensure that the rubber insulating gloves issued for employee use had been tested within the past six months since being placed into service to ensure protection at the rated voltage.  Certification was not made available to the Secretary's representative upon request.  Employees were exposed to electric shock hazards when performing electrical work.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.335(a)(1)(i): Employees working in areas where there are potential electrical hazards shall be provided with, and shall use, electrical protective equipment that is appropriate for the specific parts of the body to be protected and for the work to be performed.    On or about 03 August 2016, and on prior dates, an employee gained access to a live electrical panel to reset fuses/breakers in a 480volt live electrical panel and did not wear appropriate electrical protective equipment, including but not limited to the following, to protect against electrical shock and Arc flash/blast injuries:      a) Rubber insulating gloves with leather protectors.      b) Arc-Rated clothing such as arc-rated long-sleeve shirt and pants, or arc-rated coverall.      c) Hearing protection (canal inserts).      d) Arc-Rated flash suit, hood or balaclava.      Abatement documentation is required of this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.335(a)(1)(v): Employees shall wear protective equipment for the eyes or face wherever there is danger of injury to the eyes or face from electric arcs or flashes or from flying objects resulting from electrical explosion.    On or about 03 August 2016, and on prior dates, an employee who gained access to a live electrical panel to reset fuses/breakers in a 480volt live electrical panel, did not wear appropriate protective equipment, such as safety glasses and an arc-rated face shield,  to protect the eyes and face against electrical arc flash/blast injuries.     Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

Employees shall be trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertain to their respective job assignments.     On or about 03 August 2016, and on dates prior, employees are required to enter 480volt electrical panel to check fuses and breakers for tripping and reset if needed. The employees were not trained in and familiar with safety-related work practices, such as the use of special precautionary techniques, personal protective equipment (PPE) and insulating and shielding materials.
