29 CFR 1926.451(a)(6): Scaffolds were not constructed in accordance with the design by a qualified person: a) South side of the building, on or about August 18, 2016, two employees installing wrapping paper around the wooden structural were elevated and working from an unapproved work platform (a 4X4 wooden pallet) that was supported by a CAT TL 642C high reach forklift, thus exposing employees up to a 16-foot fall hazard.

29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied: a) South side of the building, on or about August 18, 2016, two employees were elevated and working from a 4 X 4' wooden pallet that was not secured to the forks of the CAT TL 642C high reach forklift, which exposed employees to a 16-foot fall hazard.
