29 CFR 1926.20(b)(2):  The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person:   (a) All Home Repairs, Inc. 700 Bonnie Brae Blvd, Denver, CO. 80209:  On or about July 26th, 2016, the employer did not ensure a "competent" person made frequent inspections of the jobsite to ensure the use of fall protection while employees worked on a 7 and 12 sloped church roof. Six employees were observed installing new roof decking without adequate fall protection.  This condition exposed employees to a fall hazard of approximately 20 feet to the ground below.    Abatement Note: OSHA defines a "competent" person as one who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has authorization to take prompt corrective measures to eliminate them.   Abatement Note:  Abatement certification is required for this item (see enclosed "Sample Abatement-Certification Letter").

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toe-boards, safety net systems, or personal fall arrest systems:  (a)  All Home Repairs, Inc. 700 Bonnie Brae Blvd, Denver, CO. 80209:  The employer did not provide and ensured the use of fall protection while employees installed new roof decking on a 7 and 12 sloped church roof. As a result, six employees were exposed to a fall hazard of approximately 20 feet to the ground below. 29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   (a)  All Home Repairs, Inc. 700 Bonnie Brae Blvd, Denver, CO. 80209:  The employer did not ensure fall protection training was provided by a "competent" person prior to permitting employees to work on a 7 and 12 sloped church roof. As a result, employees were exposed to a fall hazard of approximately 20 feet to the ground below.    Abatement Note:  Abatement certification is required for this item (see enclosed "Sample Abatement-Certification Letter").   Abatement Note: Upon completion of fall protection training, the employer must verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or signature of the employer.
