29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: (a) On or about October 18, 2017, employees were exposed to overhead falling object hazards while working on the ground below roofing operations without wearing a hard hat. A 2x4 with protruding nails was thrown from the roof in the area of a worker on the ground. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: (a) On or about October 18, 2017, and at times prior, workers performing roofing activities with pneumatic nail guns and adjacent to the pneumatic nail guns, were not wearing safety glasses and were exposed to eye injuries from kickback, misfire, and flying debris. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet"). All Home Repairs Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1175507, citation number 1, item number 1, and was affirmed as a final order on November 10, 2016, with respect to a workplace located at 5463 Killen Ave. in Castle Rock, Colorado, 80104.
