29 CFR 1926.102(a)(1): 29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  (a) On or about June 28, 2017, and at times prior, All Home Repairs Inc. as the employer did not ensure that employees performing roofing activities with pneumatic nailers were wearing safety glasses to prevent eye injuries due to flying debris.  Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").  All Home Repairs Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard Eye and face protection 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1175507, citation number 1, item number 1 and was affirmed as a final order on November 10, 2016, with respect to a workplace located at 5463 Killen Ave., Castle Rock, CO 80104.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  (a) On or about June28, 2017, Employees of All Home Repairs Inc. were exposed to falls of approximately 40 feet or more while engaged in reroofing activities of a senior living facility in 7100 W 13th Ave. Lakewood, CO 80214 without any means of fall protection.  Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").  All Home Repairs Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard Duty to have fall protection 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1175507, citation number 1, item number 2 and was affirmed as a final order on November 10, 2016, with respect to a workplace located at 5463 Killen Ave., Castle Rock, CO 80104.   All Home Repairs Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard Duty to have fall protection 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1189409, citation number 1, item number 1 and was affirmed as a final order on March 1, 2017, with respect to a workplace located at 1304 Whitekirk Pl., Castle Rock, CO 80104.
