29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) On or about 8/30/16 at a roof replacement project located at 47 Main Street, Silver Creek, New York; employees working on the roof were placing shingles on the roof of a three story house. The roof had a 18 in 12 pitch and a 12 in 12 pitch with eave heights of approximately 26 feet. There was no fall protection in use. NO ABATEMENT CERTIFICATION REQUIRED Buffalo Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 919589, citation number 1, item number 3 and was affirmed as a final order on 8/29/13, with respect to a workplace located at 76 Humboldt Parkway, Buffalo, New York.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) On or about 8/30/16 at a roof replacement project located at 47 Main Street, Silver Creek, New York; an extension ladder was used for access to the roof at the rear of the house. The ladder extended 18" above the eaves. ABATEMENT CERTIFICATION REQUIRED Buffalo Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 919589, citation number 1, item number 4 and was affirmed as a final order on 8/29/13, with respect to a workplace located at 76 Humboldt Parkway, Buffalo, New York.
