29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: Two employees were standing on an exterior wall between roof rafters, installing fascia at the end of the rafters below them. An employee working on the ground was exposed to struck by hazards when handing lumber materials up without wearing head protection in the event tools or lumber were to fall from above.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. Location at 5325 Twelve Oaks Road, Springfield, MO: (a) Access to Roof: Employees were exposed to fall hazards when they used a step ladder to access the roof; the step ladder was too short and did not extend three feet above the attic floor level.
