29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:   a) Residence: On or about August 18, 2016, and at times prior, workers performing roofing activities with pneumatic fasteners without safety glasses were exposed to eye injuries due to flying debris. 29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor was each employee provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a)  Residence: On or about August 18, 2016, and at times prior, workers performing roofing activities without fall protection were exposed to falling 10 to 18 feet to ground level from a 10 in 12 pitched roof.
