29 CFR 1910.137(c)(2)(viii): Electrical protective equipment was not subjected to periodic electrical tests. Test voltages and/or the maximum intervals between tests were not conducted in accordance with Table I-4 and Table I-5: a) Worksite - on September 7, 2016, and at times prior, electrical protective rubber insulating gloves were not tested every six months to ensure that the insulating properties of the equipment is maintained.

29 CFR 1910.137(c)(2)(iii)(D): Damaged electrical protective insulating equipment, that had either a hole, tear, puncture, cut, ozone cutting or cracking; an embedded foreign object; or texture changes such as swelling, softening, hardening, or became sticky or inelastic; or had any other defect that affected the insulating properties was not removed from service. a) Worksite - on September 7, 2016, and at times prior, in-service electrical protective equipment showed signs of stretching and softening that required it be removed from service.
