29 CFR 1903.19(c)(1): Within 10 calendar days after the abatement date, the employer must certify to OSHA (the Agency) that each cited violation has been abated, except as provided in paragraph (c)(2) of this section.  For Citation 1, Item 1, issued on 10/4/2016, with an abatement date of 10/11/2016, the employer did not certify the complete abatement of the cited violation.

29 CFR 1903.19(d)(1): The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  For Citation 1, Item 2, issued on 10/4/2016, with an abatement date of 10/11/2016, the employer did not submit documents demonstrating complete abatement of the cited violation to OSHA.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels and 50-feet (15.25 m) wide or less, was not protected from falling by guardrail systems, safety net systems:    The employer did not ensure appropriate fall protection was implemented for employees performing commercial roofing operations on a flat roof that was greater than 50 feet wide when:  A.  Workers were exposed to the hazard of falling approximately eight (8) feet while performing roofing operations on the roof of a garage.  B.  Workers were exposed to the hazard of falling approximately twenty (20) feet while performing roofing operations on the roof an assisted living center.
