29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of �1926.502(k) developed and implemented: The employer is failing to protect employees from fall hazards. This was most recently documented at a worksite located at or near 21971 Brookside Ave., Elkhorn, Nebraska 68022 on or about December 5, 2017. The employer did not ensure that adequate fall protection was provided for and worn at all times while working on the roof of a home which was approximately 10 feet from a lower level.
