On or about 3/21/2019, Luis Caceres DBA Luis Caceres was installing roofing materials on a roof of a residential building located at 1343 Briarchase Drive in O Fallon, MO.  The foreman and three employees were not utilizing guardrail systems, a safety net system, or personal fall arrest systems while installing roofing materials on a 7/12 sloped roof approximately 8-14 feet above the ground.  The employees were not able to recognize fall hazards and the procedures necessary to reduce or eliminate hazards.
