OSH ACT of 1970 Section (5)(a)(1): OSH ACT of 1970 Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to caught-in hazards: On or about 9/8/16 in the fabrication shop area: Employees were exposed to a laceration and amputation hazard from the Roller Winder Machine (serial no. 11013943-3944) ; the e-stop pull cable was broken and not continuously operable. Among other methods, one feasible and acceptable abatement method to correct this hazard is to follow the National Fire Protection Association (NFPA) 79, Electrical Standard for Industrial Machinery, 2012 Edition. Stop and Emergency Stop push buttons shall be continuously operable and readily accessible in accordance with the guidelines set forth in NFPA 79, Article 10.7 .1 Location and Operation.

29 CFR 1910.213(g)(1): Swing cutoff saw(s) were not provided with an automatically adjusting hood that completely enclosed the upper half of the saw, the arbor end, and the point of operation at all positions of the saw: On or about 9/8/16 in the pallet shop outside area: Employees were exposed to a laceration and amputation hazard from the 12-inch Chop saw where the guard on the bottom was missing.
