On 12/10/2019, at the Elberton facility, the employer did not develop, document and implement specific written Lockout procedures for equipment employees were servicing and maintaining. Equipment includes but is not limited to presses, ladder machines and saws.

On 12/10/2019, at the Elberton facility, employees were conducting service and maintenance to production equipment at the facility and the employer has not conducted training to those performing Authorized employee work in the hazards of that work.
