Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a.) 50th Street, Charleston WV: On or about 08/19/16, employees installing a new roof on a low slope roof were not protected from falling 17'6" to the ground below.
