29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: The employer is failing to ensure that all employees exposed to fall hazards while working on a residential roof are utilizing fall protection. This most recently occurred on September 12, 2016 on the jobsite located at 6624 Old Dominion Rd, Lincoln, NE 68516. Employees were observed removing roofing materials on a 12:12 pitched residential roof at heights greater than 6 feet above the ground and lower levels.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. The employer is failing to ensure that all employees are protected from fall hazards when entering/exiting the landing surface of a residential roof. This most recently occurred on September 12, 2016 on the jobsite located at 6624 Old Dominion Rd, Lincoln, NE 68516. Employees were observed using ladders that did not have side rails which extended to at least 3 feet above the landing surface when entering/exiting the roof to remove roofing materials.
