29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system; The employer is failing to protect employees from a fall hazard. This was most recently observed on October 29, 2018 at or about 827 N 195th St. Omaha, Nebraska. The employer failed to ensure that employees working on a roof at a height of 10 feet 3 inches as measured from the eve to the ground and a pitch of 4:8 were utilizing fall protection. The Leo Garcia Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number1177109, citation number1, item number 1 and was affirmed as a final order on October 25, 2016, with respect to a workplace located at 6624 Old Dominion Road Lincoln, Nebraska.

This was most recently documented on October 29, 2018 at or about 827 N. 195th St. Omaha, Nebraska. The employer failed to ensure that the ladder being used to access the roof was extended at least 3 feet above the eve as required by this sub-part. The Leo Garcia Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1177109, citation number1, item number 2 and was affirmed as a final order on October 25, 2016, with respect to a workplace located at6624 Old Dominion Rd.
