29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    On or about September 6, 2016, while employee(s) were setting-up, adjusting and unjamming (servicing) the HC machine including removing a piece of plastic at the knife area while machine was running, the employer did not require the application of:     (a) Turn off and shut down the machines,  (b) Physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source,  (c) Affix lockout or tagout devices to each energy isolating device,  (d) Verify that isolation and deenergization of the machine or equipment have been accomplished.    Employees were exposed to caught-in hazards likely to result in amputation of hand(s) and fingers.    The employer was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1114530, citation number 1, item number 1 and was affirmed as a final order on March 8, 2016, with respect to a workplace located at 1703 N. Randall Road in Elgin, Illinois 60123.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(c)(7)(iii)(B): Additional retraining shall also be conducted whenever a periodic inspection under paragraph (c)(6) of this section reveals, or whenever the employer has reason to believe that there are deviations from or inadequacies in the employee's knowledge or use of the energy control procedures.  The employer did not ensure that retraining was provided to affected employees whenever there were possible inadequacies in the affected employees knowledge of the energy control procedure.

29 CFR 1910.147(c)(4)(ii)(D): Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.  The employers energy control procedure for the HC machine did not fully document the alternative measures which provide effective protection to employees during minor servicing activities while operating the machine in bypass mode.
