29 CFR 1910.178(l)(4)(ii)(B): Refresher training in relevant topics was not provided to the operator when the operator was involved in an accident or near-miss incident. a) On or about September 16, 2016, or prior thereto: The employer did not perform refresher training for all employees involved in accident or near-miss incidents. This was most recently observed when an operator on a Powered Industrial Truck (PIT) was involved in an accident on September 8, 2016 at Debox at Line 6. The operator caught a table with the PIT and subsequently hit an employee with a pallet of cheese. NOTE: This item was corrected during the inspection.
