OSH ACT of 1970 Section (5)(a)(1) The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees 29 CFR 1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about 02/10/2025, in the Bulk Mozzarella Packaging Area, lockout/tagout procedures were not utilized when an employee cleaned the guide rail of a running conveyor belt. The employee was exposed to a pinch point between the guide rail and the moving belt. The employee sustained a fingertip amputation. b) On or about 02/13/2025, in the Bulk Mozzarella Packaging Area, when employees performed setup and changeover of the New Douglas and Schneider case packer which required the opening of guards and adjustment of parts without locking out or controlling energy sources, employees were exposed to caught-by hazards in the event of unexpected machine start-up while reaching inside to adjust components. ABATEMENT DOCUMENTATION REQUIRED
