29 CFR 1910.24(b): Fixed stairs were not provided for access from one structure level to another where operations necessitated regular travel between levels, and for access to operating platforms at any equipment which requires attention routinely during operations:  a) Callaway Farms Manufacturing LLC, Washington, GA - Employees were exposed to fall hazards of over 8 feet from working at the outside log deck platform. Employees stepped out the stairs located next to the conveyor and onto the platform. Fall protection was not provided while accessing the platform from the stairs. Employees were accessing the platform 3-4 times per day, on or about 9/26/16.  b) Callaway Farms Manufacturing LLC, Washington, GA - Employees were exposed to fall hazards of over 8 feet from working at the outside log deck platform.  Employees went over the stairs railing and onto the platform. Fall protection was not provided while accessing the platform from the stairs. Employees were accessing the platform 3-4 times per day, on or about 9/26/16.

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toe boards.  a) Callaway Farms Manufacturing LLC, Washington, GA - Employees were exposed to fall hazards of over 8 feet from working at the outside log deck platform. The platform was not equipped with standard guardrails. Employees stood on the platform to move logs that were jammed, on or about 9/26/16.  b) Callaway Farms Manufacturing LLC, Washington, GA - Employees were exposed to fall hazards of over 8 feet from working at the outside log deck platform. Employees were stepping from the stairs to the platform in an area that was not protected with standard guardrails, on or about 9/26/16.  Callaway Farms Manufacturing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.23(c)(1), which was contained in OSHA inspection number 1106538, citation number 1, item number 2 and was affirmed as a final order on 12/21/15, with respect to a workplace located at 371 Harris Rd, Washington, GA.

29 CFR 1910.95(b)(1): When employees were subjected to sound exceeding those listed in Table G-16, feasible administrative or engineering controls were not utilized:  a) Callaway Farms Manufacturing LLC, Washington, GA - A log deck operator was exposed to projected noise level of 103 dBA, exceeding the permissible exposure limit (PEL) of 90 dBA. The exposure was derived from a sample taken over 404 minutes. Effective engineering controls were not utilized, on or about 9/26/16.  b) Callaway Farms Manufacturing LLC, Washington, GA - A log deck operator was exposed to a projected noise level of 100 dBA, exceeding the permissible exposure limit (PEL) of 90 dBA. The exposure was derived from a sample taken over 406 minutes. Effective engineering controls were not utilized, on or about 9/26/16.  Callaway Farms Manufacturing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.95(b)(1), which was contained in OSHA inspection number 1105640, citation number 1, item number 2 and was affirmed as a final order on 12/21/15, with respect to a workplace located at 371 Harris Rd, Washington, GA.  General methods of control applicable in these circumstances include, but are not limited to the following:  1.  Isolate the employee operating the log deck at the computer station in a temperature control booth or enclosure.  2.  Explore the possibility of using administrative controls by decreasing the amount of hours the employees are exposed to during the work day.  However, other methods of abatement may be equally, or more appropriate.  Ultimate responsibility for determining the most appropriate abatement method rests with the employer given its superior knowledge of the specific conditions at its worksite.  Abatement Schedule  Step 1 -   A written detailed plan of abatement shall be submitted to the Area Director outlining a schedule for the implementation of engineering and/or administrative measures to control employee exposures to hazardous substances as referenced in this citation. This plan shall include, at a minimum, target dates for the following actions which must be consistent with the abatement dates required by this citation:  a) Evaluation of engineering/administrative control options; b) Selection of optimum control methods and completion of design; c) Procurement, installation and operation of selected control measures; d) Testing and acceptance or modification/redesign of controls.  All proposed control measures shall be approved for each particular use by a competent industrial hygienist or other technically qualified person. 60-day progress reports are required during the abatement period.  Step 2: Abatement shall have been completed by the implementation of feasible engineering and /or administrative controls upon verification of their effectiveness in achieving compliance.
