29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  On or about September 15, 2016, at Aspen Products, Inc., 3220 Avondale Mill Road, Macon, Georgia, production floor:  a) Employees performed servicing of Peerless Model E-57 Presses to include, but not limited to clearing jams without utilizing Lockout/tag out procedures were exposed to amputation hazards and one employee was injured amputating his right middle finger while clearing a jam from the Peerless Model E-57, Serial Number E5-1004-15, when lockout/tag out procedures were not utilized.   b) An employee performed servicing of a Peerless Model E-57, Serial Number E5-1012-15 without utilizing Lockout/tag out procedures was exposed to amputation hazards when he stood inside the press to clear a jam and thread the paper into the press.
