29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: (a) Jobsite: On or about September 22, 2016, at lease three employees were not protected from falling from a flat roof. The employees were replacing the flat roof. The fall hazard to the three employees ranged from about 10 feet to about 35 feet.

29 CFR 1926.503(c)(3): When the employer has reason to believe that any affected employee who has already been trained does not have the understanding and skill required by paragraph (a) of this section, the employer shall retrain each such employee: (a) Jobsite: On or about September 22, 2017, the employees working on the roof need to be retrained in when and how to use fall protection when working from a roof.
