29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Jobsite, 1654 Ellsworth Avenue, Heidelberg, PA:  On or about June 10, 2021, five employees were tearing off shingles on a residential roof at heights up to approximately 22 feet above the ground and were not utilizing fall protection.    Holzer Jesko Quality Exteriors, LLC  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 1318945, citation number 2, item number 1 and was affirmed as a final order on 7/11/18, with respect to a workplace located at 1819 Ellsworth Avenue, Heidelberg, PA.  Holzer Jesko Quality Exteriors, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 1179077, citation number 1, item number 1a and was affirmed as a final order on 10/13/16, with respect to a workplace located at 111 Boulevard of the Allies, Pittsburgh, PA.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) Jobsite, 1654 Ellsworth Avenue, Heidelberg, PA:  On or about June 10, 2021, employees were using a 24 foot Werner extension ladder as access and egress to the roof that was only 24 inches above the roof.   Holzer Jesko Quality Exteriors, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1318945, citation number 1, item number 3 and was affirmed as a final order on 7/11/18, with respect to a workplace located at 1819 Ellsworth Avenue, Heidelberg, PA.

29 CFR  1926.503(b)(1): The employer shall verify compliance with paragraph (a) of this section by preparing a written certification record. The written certification record shall contain the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer. If the employer relies on training conducted by another employer or completed prior to the effective date of this section, the certification record shall indicate the date the employer determined the prior training was adequate rather than the date of actual training:  a) Jobsite, 1654 Ellsworth Avenue, Heidelberg, PA:  On or about June 10, 2021, two employees were not on the fall protection training records the company provided and they did not provide verification of the training they said they received from a former employer.  Holzer Jesko Quality Exteriors, LLC  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(b)(1), which was contained in OSHA inspection number 1318945, citation number 3, item number 1 and was affirmed as a final order on 7/11/18, with respect to a workplace located at 1819 Ellsworth Avenue, Heidelberg, PA  15106.

29 CFR  1926.503(c)(3):Inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill:    a) Jobsite, 1654 Ellsworth Avenue, Heidelberg, PA:  On or about June 10. 2021, employees were working on a residential roof at heights up to approximately 22 feet above he ground without fall protection.  The owner provided the fall protection training records for them that were dated in April.  Holzer Jesko Quality Exteriors, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(c)(3) which was contained in OSHA inspection number 1179077, citation number 1, item number b and was affirmed as a final order on 10/13/2016, with respect to a workplace located at 111 Boulevard of the Allies, Pittsburgh, PA  15222.
