29 CFR 1926.502(f)(1)(ii): Where mechanical equipment was being used, the warning line was not erected not less than 6 feet (1.8 m) from the roof edge which was parallel to the direction of mechanical equipment operation, and not less than 10 feet (3.1 m) from the roof edge which was perpendicular to the direction of mechanical equipment operation:   a) Job site, 1819 Ellsworth Avenue, Carnegie, PA: On or about May 29, 2018, during roofing activities on the upper roof, the employer did not ensure that flag lines being used for fall protection were properly placed a safe distance from the edge of the roof. A cutter machine was being operated perpendicular to the 3 existing flag lines which were determined to be between approximately 2 to 5 feet from the edges of the roof, exposing employees to fall hazards of up to at least approximately 18 feet.

Holzer Jesko Quality Exteriors, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1179077, citation number 1, item 1a, and was affirmed as a final order on October 13, 2016, with respect to a workplace located at 111 Boulevard of the Allies, Pittsburgh, PA.
