29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition: a) On the 4th floor, near the shaker machinery, collected piles of dust and debris were observed on floors, on top of machinery and lighting fixtures, on or about September 23, 2016. The Strategic Materials was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.22 (a)(1), which was contained in OSHA inspection number 1105342, citation number 02 item number 001 and was affirmed as a final order on 06/03/2016, with respect to a workplace located at 6420 19th St., E. Sarasota, FL 34243. The Strategic Materials was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.22 (a)(1), which was contained in OSHA inspection number 906337, citation number 01 item number 001 and was affirmed as a final order on 11/17/2014, with respect to a workplace located at 2728 Buell Drive East Troy, WI 53120.

29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user: (a) For employees using a 3M, model 6200 half mask respirator with 7093 particulate cartridges while checking quality of product being produced from the second floor conveyor, on or about September 23, 2016.
