In the following instances (a-c): a) On April 4, 2025, an employee wore a balaclava and hoodie while using a tight-fitting respirator; b) On April 17, 2025, an employee wore a balaclava and hoodie while using a tight-fitting respirator; and c) On April 30, 2025, an employee wore a balaclava and hoodie while using a tight-fitting respirator.

29 CFR 1910.134(g)(1)(i)(A): Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function: On or about April 4, 2025, and at times prior thereto and thereafter, employee(s) were observed having facial hair while using a required tight-fitting respirator when working in the 12-Mesh building. In the following instances (a-c): a) On April 4, 2025, an employee had facial hair while using a tight-fitting respirator; b) On April 17, 2025, an employee had facial hair while using a tight-fitting respirator; and c) On April 30, 2025, an employee had facial hair while using a tight-fitting respirator. Strategic Materials, Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.134(g)(1)(i)(A), which was contained in OSHA inspection number 1672461, Citation Number 1, Item number 1-c, and was affirmed as a final order on November 15, 2023, with respect to a workplace located at 3240 Robinson Road, Midlothian, TX 76065.

29 CFR 1910.1000(a)(2): Employee(s) were exposed to an airborne concentration of particulates not otherwise regulated (PNOR) respirable fraction listed in Table Z-1 in excess of the 8 hour Time Weighted Average concentration of 5 milligrams per cubic meter (mg/m3): On or about April 30 2025, and at times prior thereto, in the 12-Mesh building, in an 8-hour time-weighted average (TWA), an employee was exposed to particulates not otherwise regulated (PNOR) respirable fraction at concentrations of approximately 9.3 mg/m3 of air, which is approximately two (2) times the permissible exposure limit (PEL). Strategic Materials, Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.1000(a)(2), which was contained in OSHA inspection number 1672461, Citation Number 1, Item number 4, and was affirmed as a final order on November 15, 2023, with respect to a workplace located at 3240 Robinson Road, Midlothian, TX 76065.
