29 CFR 1910.147(c)(7)(i)(A): Each authorized employee had not received training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control:  Building No. 8 - On or about October 7, 2016, in the facility an employee (helper) was required to assist the maintenance supervisor with replacing a steel roll on the Bubble Machine #3. The employee (helper) was not provided training to ensure that as an authorized employee he had acquired the knowledge and skills required for the safe application, usage and removal of the energy control devices while performing maintenance and/or service on the Bubble Machine #3. The employee (helper) was thereby exposed to the hazards associated with the  inadvertent start-up of the equipment and/or release of stored hazardous energy.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    Building No. 8 - On or about September 13, 2016, in the facility employee(s) working with Bubble Machine No. 3 were required to make adjustments to the machine's hooks (self- made shim). When making the adjustments, the employee(s) were approximately two inches from the in-running side of the rotating rolls ( embossing roll and rubber nip roll) that were not guarded. Employees were thereby exposed to the hazards ( lacerations, amputations) associated with rotating parts.
