29 CFR  1910.147(c)(6)(i)(C): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed:  Where lockout was used for energy control, the periodic inspection did not include a review, between the inspector and each authorized employee, of that employee's responsibilities under the energy control procedure being inspected.   a) Plant/Building #8- On or about June 6,  2021, an authorized employee was required to perform servicing on the Poly-Cell Film Making Bubble Machine #2 Model#PCS-1930HPA. The employer did not include a review, between the inspector and each authorized employee,  of that employee's responsibilities under the energy control procedure being inspected.

29 CFR  1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  (a) Plant #8 - On or about June 6, 2021, an employee was exposed to machine hazards associated with exposed and live wires when the employee was clearing plastic debris from the  Poly-Cell Film Making Bubble Machine #2 Model#PCS-1930HPA. The employer failed to implement energy control application steps as the machine power was not shut down to perform servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], Lockout/Tagout device application [(d)(4)], dissipation of residual energy [(d)(5)((i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.

29 CFR  1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  a) On or about June 6, 2021, the employer did not contact OSHA within 24 hours of when an employee sustained a work-related injury, suffered an electric shock injury with exposed and live wires, that caused an employee an in-patient hospitalization resulting loss of two fingers to the right hand.
