The double-door exit route was equipped with a panic bar on the east door which was partially obstructed by the containers. 29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about September 27, 2016, employees were exposed to machine hazards associated with moving parts when they engaged in tool change activities on the Davenport and Acme screw machines in the Machine Shop. The employer did not ensure that Machinist employees implemented energy control application steps prior to allowing them to reach into the Davenport and Acme screw machines to conduct tool changes. The machines were not deenergized prior to the employees engaging in the tool changes. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine servicing hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Fifteen (15) Davenport screw machines located in the Machine Shop were not provided with guarding to protect the Machinists from exposure to points of operation, ingoing nip points and rotating parts in the milling enclosure portions of the machines. Machinist employees were required to engage the machines' clutches within the milling enclosures which put their hands within 8 inches to 15 inches of moving parts. b) Fifteen (15) Davenport screw machines located in the Machine Shop were not provided with guarding to protect the Machinists from exposure to rotating parts at the bar stock loading end of the machines. Machinist employees could be exposed to rotating parts of the bar stock turret or to moving stock loaded into the bar stock turret. c) Nine (9) Acme screw machines located in the Machine Shop were not provided with guarding to protect the Machinists from exposure to points of operation, ingoing nip points and rotating parts in the milling enclosure portions of the machines. Machinists would adjust oil lines within the milling enclosure while the machine was operating which put their hands within 3 inches to 6 inches of moving parts. Acme #5 was equipped with interlocks on the mill enclosure; however, employees were able to bypass the interlock to adjust the oil lines during operation. d) Seven (7) Acme screw machines located in the Machine Shop were not provided with guarding to protect the Machinists from exposure to ingoing nip points and rotating parts of the bar stock turret-drive portion of the machines. Machinist employees were required to operate the collet lever located adjacent to the moving equipment which put their hands within 2 feet to 3 feet of moving parts. e) Eight (8) Acme screw machines located in the Machine Shop were not provided with guarding to protect the Machinists from exposure to rotating parts of the bar stock turrets of the machines. Acme machines (#2, 4, 6, 7, 9, and 10) had incomplete guarding which did not protect the bottom side or the bar stock loading end of the bar stock turrets. Acme machine #3 was equipped with a fully enclosed the bar stock turret and loading end; however, the enclosure was not interlocked and employees could open the enclosure and be exposed to the rotating turret and/or bar stock in the turret during operation.

The working space was occupied by the Chip Spinner machine, its oil drain pipe, 5 gallon buckets and a drip tray.
