29 CFR 1910.119(f)(1)(i)(D): The employer shall develop and implement written operating procedures that provide clear instructions for safely conducting activities involved in each covered process consistent with the process safety information and shall address emergency shutdown including the conditions under which emergency shutdown is required, and the assignment of shutdown responsibility to qualified operators to ensure that emergency shutdown is executed in a safe and timely manner: On or about April 17, 2017 at a Worthington Industries facility located at 300 East Breed Street, Chilton, WI 53014 an incident occurred when the North Propane Tank (PV-TK03) was filled beyond its allowable 86% fill by volume. A Maintenance employee monitored the North Propane Tank as it reached at least 92% full. The employer had an emergency shutdown operating procedure; however, the employer's operating procedure did not address the conditions under which emergency shutdown is required, and the employer did not assign responsibility to qualified operators (the Maintenance employee) to ensure that emergency shutdown was executed in a timely manner.

29 CFR 1910.119(g)(1)(i): Each employee presently involved in operating a process, and each employee before being involved in operating a newly assigned process, shall be trained in an overview of the process and in the operating procedures as specified in paragraph (f) of this section. The training shall include emphasis on the specific safety and health hazards, emergency operations including shutdown, and safe work practices applicable to the employee's job tasks: On or about April 17, 2017 at a Worthington Industries facility located at 300 East Breed Street, Chilton, WI 53014 an incident occurred when the North Propane Tank (PV-TK03) was filled beyond its allowable 80% fill by volume (as determined by the employer) and beyond 86% (according to NFPA 58). A Maintenance employee monitored the North Propane Tank as it reached at least 92% full. The employer did not train the Maintenance employee on emergency operations such as but not limited to; how to react to fill levels beyond 80-86%, and conditions that require an emergency shutdown.
