29 CFR  1910.333(b)(2):While an employee was exposed to contact with parts of fixed electric equipment or circuits which were deenergized, the circuits energizing the parts were not locked out or tagged or both in accordance with the requirements of this paragraph (b) of 29 CFR 1910.333(b)(2):  a.)	On or about March 2, 2021 the employer did not ensure the Galv Line electrical cabinet in the MCC Room was deenergized and locked out prior to troubleshooting loose terminals, and were exposed to electric shock and arc flash hazards.

29 CFR  1910.333(c)(2):The employer did not ensure that only qualified persons were allowed to work on energized circuit parts or equipment that had not been de-energized under the procedures of paragraph (b) of this section.  A)	On or about March 2, 2021, the employer failed to ensure that maintenance employees, who conducted electrical troubleshooting and tightening loose terminals in the Galv Line cabinet in the MCC Room, were qualified to work on electric circuit parts or equipment that have not been deenergized. Employees were exposed to electrical and arc flash hazards associated with 480 volts.

29 CFR  1910.335(a)(1)(i):Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed: a.)	On or about March 9, 2021, the employer did not ensure electricians were protected while performing work on or near 480V circuits. Employees were required to troubleshoot loose connections for the Galv Line electrical cabinet in the MCC Room and did not wear required hand and body protection while performing live electrical work, exposing them to electrical shock and arc flash hazards.

29 CFR  1910.332(b)(1):Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments:  a.)	On or about March 2, 2021, the employer did not ensure employees were familiar with electrical safety related work places before performing work on 480V circuits. Employees were required to troubleshoot loose connections for the Galv Line electrical cabinet in the MCC Room and did not have a full understanding of working with live 480V terminals.
