29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    (a) New residential construction, Rear roof: On or about October 3, 2016, and at times prior, employees were exposed to fall hazards of 20-feet, 3-inches to 29-feet, 8-inches to the lower level while installing a chimney cap, in that employees failed to use a personal fall arrest system to prevent falls to the lower level. One of the exposed employees was not trained by the employer to recognize the hazards of falling and the procedures to follow to minimize these fall hazards.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:     (a)  New residential construction, Rear roof: On or about October 3, 2016, and at times prior, the employer exposed employees to fall hazards over 20-feet while working from a 8/12 pitched roof installing a chimney cap, in that a personal fall arrest system was not used during installation. Employees were exposed to falls up to 29-feet, 8-inches to the lower ground level.     M.R. Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 977426, citation number 1, item number 1 and was affirmed as a final order on July 8, 2014, with respect to a workplace located at Lot #13 Northwynn Subdivision in Sharpsburg, GA.
