29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition:    (a)  Little Colorado, LLC, 4450 Lipan Street, Denver, CO  80211:  On and before September 30, 2016, the employer did not ensure that the shop and wood-cutting operations areas were kept clean.  The employer operated several CNCs and other wood-cutting equipment that generated combustible dust.  The employer did not establish appropriate housekeeping procedures to prevent the buildup of combustible dust on the floors, walls, and electrical equipment.  This condition exposed employees to fire hazards.

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, liquid chemicals, acids or caustic liquids, chemical gases or vapors:    (a)  Little Colorado, LLC, 4450 Lipan Street, Denver, CO  80211:  On and before September 30, 2016, the employer did not ensure that employees wore appropriate eye or face protection when operating or working near equipment in the cutting, shaping, and finishing operations.  This condition exposed employees to eye and face injury hazards from projectiles to the eyes and from sharp edges.    (b)  Little Colorado, LLC, 4450 Lipan Street, Denver, CO  80211:  On and before September 30, 2016 the employer did not ensure that employees wore appropriate eye or face protection when employees were painting furniture in the spray booths and painting areas. This condition exposed employees to eye and face injury hazards from being sprayed with paint products.

29 CFR 1910.212(a)(3)(ii): Points of operation of machinery were not guarded to prevent employees from having any part of their body in the danger zones during operating cycles:    (a)  Little Colorado, LLC, 4450 Lipan Street, Denver, CO  80211:  On and before September 30, 2016, the employer did not ensure that points of operation of machinery were guarded.  The employer operated the CNC machines numbers 1 and 2, and the points of operation where the drilling occurred on the open work platform were not guarded.  In addition, the work platform holding the material moved laterally, with the outer perimeter of the material presenting a struck-by hazard for employees operating the machine or for employees walking in close proximity to this moving platform.  These conditions exposed employees to amputation and struck-by hazrds.
