29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a. On or about October 5, 2016, employees performed servicing or maintenance on equipment around the shop such as the ZDCQ bridge cutting machine and the compressors located behind the building and the employer did not establish a written lockout/tagout program that consisted of written specific instructions on how to lockout each piece of equipment , employee training and periodic inspections, exposing the employees to amputation hazards.   Stone House Enterprises, Inc. dba Jacksonville Granite was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(1), which was contained in OSHA inspection number 1130665, citation number 01, item number 002 and was affirmed as a final order on July 25, 2016, with respect to a workplace located at 124 Industrial Loop W, Orange Park, FL 32073.
