29 CFR 1910.157(e)(3): Portable fire extinguishers were not subjected to an annual maintenance check:  a. On or about December 20, 2018, in office area, the annual maintenance check a portable fire extinguishers was last conducted in May of 2017, and was more than 6 months delinquent in receiving the required annual maintenance check.  b. On or about December 20, 2018, in the fabrication shop area, the annual maintenance check of four portable fire extinguishers was last conducted in May of 2017, and was more than 6 months delinquent in receiving the required annual maintenance check.  c. On or about December 20, 2018, outdoors and near the LP gas storage rack, the annual maintenance check of a portable fire extinguishers was last conducted during the year of 2016, and was nearly 2 years delinquent in receiving the required annual maintenance check.  Stone House Enterprises, Inc. d/b/a Jacksonville Granite was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.157(c)(1), which was contained in OSHA inspection number 1181611, citation number 02, item number 001 and was affirmed as a final order on March 7, 2017, with respect to a workplace located at 124 Industrial Loop W, Orange Park, FL 32073.  Stone House Enterprises, Inc. d/b/a Jacksonville Granite was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.157(c)(1), which was contained in OSHA inspection number 1130665, citation number 02, item number 002(a) and was affirmed as a final order on July 25, 2016, with respect to a workplace located at 124 Industrial Loop W, Orange Park, FL 32073.

29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial truck:  a. On or about December 20, 2018, and at times previous, the employer did not ensure that an employee who operated a Toyota brand forklift truck, to move granite stone and to load and/or unload trucks was, trained prior to that employee operating the truck and exposed that employee and three other shop employees to struck-by hazards.  The Stone House Enterprises, Inc., doing business as Jacksonville Granite was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(l)(1)(ii), which was contained in OSHA inspection number 1130665, citation number 2, item number 3 and was affirmed as a final order on July 25, 2016, with respect to a workplace located at 124 Industrial Loop W., Orange Park, Florida 32073.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  a. On or about December 20, 2018, and at times previous,  the employer failed to provided effective information and training on hazardous chemicals to four employees who used, stored, transported, and or, worked in the vicinity of hazardous chemicals such as, but not limited to adhesives, acetone, silicon, lubricating oils, hardening agents, caulking, and granite and quartz stone dusts, while working in the fabrication area of the shop and at temporary worksites, thereby exposing the employees to physical injury and respiratory irritation and or illnesses.  Stone House Enterprises, Inc. d/b/a Jacksonville Granite was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.1200(h)(3)(iv), which was contained in OSHA inspection number 1132304, citation number 01, item number 002.b. and was affirmed as a final order on May 2, 2016, with respect to a workplace located at 124 Industrial Loop W, Orange Park, FL 32073.
