29 CFR 1926.20(b)(2): Frequent and regular inspections of the job site, materials, and equipment to be conducted by a competent person designated by the employer was not provided by the employer who was responsible to initiate and maintain an accident prevention program:  a- Block 14802, Lot 89, 6 Mimosa Court, Sicklerville NJ: A competent person did not conduct frequent and regular inspections of the jobsite.  Employees on this site were exposed to hazards including, but not limited to fall and struck by hazards.  Condition noted on or about 10/06/16.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  a- Block 14802, Lot 89, 6 Mimosa Court, Sicklerville NJ: a company employee, not wearing head protection, was exposed to struck by hazards at the worksite when working beneath another company employee who was using unsecured hand tools, pneumatic nailers, etc.  And a company employee was exposed to fall hazards when accessing and egressing the roof of a residential structure using its interior wall frames and exterior roof trusses.  At the time of this worksite inspection the employer had not provided these employees with any training or instruction regarding the recognition and avoidance of these unsafe conditions.  Condition noted on or about 10/06/16.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:  a- Block 14802, Lot 89, 6 Mimosa Court, Sicklerville NJ: A company employee, not provided with stairways/ladders, etc. was exposed to fall hazards, in excess of 9 feet to the floor below when accessing and egressing the roof of a residential structure by using the structure's vertical and horizontal framing.  Condition observed on or about 10/06/16. 29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  Each employee engaged in framing and other related activities is not protected from head injury by using protective helmets.  Quick Carpentry, Inc. does not ensure the use of head protection/protective helmets while performing framing and other related activities at the worksite. This violation was most recently observed at Block 14802, Lot 89, 6 Mimosa Court, Sicklerville NJ where a company employee, not wearing head protection, was exposed to struck by hazards when working beneath a company employee using unsecured hand tools, pneumatic nailers, etc.  This condition was observed on or about 10/06/16.  To abate this violation the employer must ensure that its employees performing framing and other activities, as well as exposed to struck by hazards, are using protective helmets.   Quick Carpentry, Inc. was previously cited for a Serious violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a), which was contained in OSHA inspection number 1138364, citation number 1, item number 1 and was affirmed as a final order on 08/01/16, with respect to a workplace located at Block 404.37, Lot 2, 2 Kendall Drive, Medford NJ.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   Each employee engaged in framing activities (with unprotected sides and edges) and when working/walking next to unguarded window openings is not protected from falling by the use of fall protection system(s).  Quick Carpentry, Inc. does not ensure the use of fall protection while performing framing work and when walking and working next to unguarded window openings.  This violation was most recently observed at Block 14802, Lot 89, 6 Mimosa Court, Sicklerville NJ where for instance "A" a company employee was exposed to fall hazards, in excess of 21 feet to the ground surface below, when performing framing and other related activities without the use of fall protection and for instance "B" a company employee was exposed to fall hazards, in excess of 13 feet to the ground surface below, when walking/working next to an unguarded wall opening.  The conditions for both instance "A" and "B" were noted on or about 10/06/16.  To abate this violation the employer must ensure that its employees performing framing work, and other related activities, are protected from falls and that no employee is allowed to be exposed to a fall hazard of 6 feet or more without the use of adequate fall protection.   Pursuant to 29 CFR 1903.19, Quick Carpentry, Inc. must submit documents describing the steps it is taking to ensure compliance, including a description of how these steps protect its employees from fall hazards.   Quick Carpentry, Inc. was previously cited for a Repeat violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1147849, citation number 1, item number 1 and was affirmed as a final order on 08/01/16, with respect to a workplace located at 25th & Atlantic Avenue, Longport NJ.

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii)  Each employee performing framing and other related activities, in excess of six feet above the ground surface below, is not trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii).  Quick Carpentry, Inc. does not ensure that each employee working at heights greater than six feet above the ground surface below has been trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii).  This violation was most recently observed at Block 14802, Lot 89, 6 Mimosa Court, Sicklerville NJ where company employees were exposed to fall hazards up to and in excess of 21 feet to the ground surface below when performing framing and other related activities from pitched roofs and by unprotected window openings without the use of any fall protection.  At the time of the inspection, these employees had not received training regarding the identification, elimination, etc. of fall hazards by a competent person.  This condition was noted on or about 10/06/16.  To abate this violation the company must ensure that all of their employees who work at heights greater than six feet above the ground surface below, have been trained in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii).
