29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s). On or about October 11, 2016: Employees operating powered industrial trucks in the freezer, cooler and dock areas are exposed to struck-by and other hazards where powered industrial trucks such as but not limited to Komatsu Forklifts (Model #FB25 S-5, Serial #35006A and Model #FB25 SH-5, Serial #36161A) are not taken out of service when repairs are needed such as but not limited to: a. Horn was not operational or missing - Komatsu Forklift #22 and Komatsu Forklift #24 b. No back-up alarm - Komatsu Forklift #22 and Komatsu Forklift #24 c. Left tilt cylinder hydraulic leak - Komatsu Forklift #24 d. No lights (headlights or rear lights) - Komatsu Forklift #22 and Komatsu Forklift #24 e. Worn tires - Komatsu Forklift #22 f. Overhead guard, damaged - Komatsu Forklift #22 and Komatsu Forklift #24 g. Gauges/Dashboard, damaged/missing - Komatsu Forklift #22 and Komatsu Forklift #24 h. Battery connect cable and housing damaged - Komatsu Forklift #22
