29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):   Employees who performed servicing and maintenance on and around the debarking conveyors were not required to:  (1)  know the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy,  (2)  turn off and shut down the machines,  (3)  physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source,  (4)  affix lockout or tagout devices to each energy isolating device,  (5)  prior to starting work on machines that have been locked out or tagged out, verify that isolation and deenergization of the machine or equipment has been accomplished.     On or about August 18, 2016, in the Debarker Area, employee(s) were cleaning bark from the out-feed of the bark discharge conveyor (Kafka Conveyors, S/N:  EL2450406375T) without performing the necessary steps to ensure proper de-energization.  This exposed employees to unexpected start-up and energization of the conveyor.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a)  In the Debarker Area, the 4' bark conveyor had an in-running nip point where the belt rotated around the drum exposing employees to caught in hazards.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours, either by telephone or in person to the OSHA Area Office that is nearest to the site of incident, by telephone to the OSHA toll-free central telephone number, 1-800-321-OSHA (1-800-321-6742), or by electronic submission using the reporting application located on the OSHA public web site at www.osha.gov:   (a)  On or about August 18, 2016, an employee was hospitalized following a work-related injury.  The employer did not report the hospitalization within 24-hours.
