29 CFR 1910.219(f)(3): Sprockets and chains.  All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform.  Where the drive extends over other machine or working areas, protection against falling shall be provided.  This subparagraph does not apply to manually operated sprockets.  a.  On March 10, 2021, an employee suffered a partial amputation of the right thumb from an unguarded chain and sprocket on the Pendu Board Dealer on the four foot picket line.   b.  On or about April 20, 2021, employees working around the Pendu Lugged Unscrambler were exposed to ingoing nip points and caught-in hazards created by an unguarded chain and sprocket mounted underneath the infeed deck on the four foot picket line.   c.  On or about April 20, 2021, employees working around the transfer decks between the hew saw and six foot trim saw were exposed to ingoing nip point and caught-in hazards created by an unguarded chain and sprocket mounted underneath the transfer deck.

29 CFR  1910.303(b)(7)(iv):There shall be no damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating.  a.  On or about April 20, 2021, the employer did not ensure that electrical conductors were protected from abrasion by broken conduit that was mounted on a beam by the Debarker.    b.  On or about April 20, 2021, the employer did not ensure that electrical conductors were protected from abrasion by broken conduit mounted on the conveyor leading to the Dog Ear Machine on the four foot picket line.

29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded.  In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy.  Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.  a.  On or about April 20, 2021, a receptacle box that was located in the debarker office used for the coffee maker did not have a faceplate.   b.  On or about April 20, 2021, a circular junction box that was located on the north wall of the Hew Saw Room used to power exhaust fans did not have the cover in place.

29 CFR  1910.305(g)(1)(iv): Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used:   a.  On or about April 20, 2021, the employer utilized a flexible cord as a substitute for fixed wiring for a light in the debarker area.   b.  On or about April 20, 2021, the employer utilized extension cords and flexible cords as a substitute for fixed wiring to power a portable heater by the 6 foot trim saw in the new addition.    c.  On or about April 20, 2021, the employer utilized an extension cord as a substitute for fixed wiring to power a radio on the northeast wall of the new addition.

29 CFR  1910.305(g)(2)(ii): Flexible cords may be used only in continuous lengths without splice or tap. Hard-service cord and junior hard-service cord No. 14 and larger may be repaired if spliced so that the splice retains the insulation, outer sheath properties, and usage characteristics of the cord being spliced.  On or about April 20, 2021, the employer had a conductor powering a light fixture in the debarker area that was not a continuous length.  The flexible cord had two splices prior to connecting the light fixture.
