29 CFR 1910.147(a)(3)(i): The employer did not utilize procedures for affixing appropriate lockout devices or tagout devices to energy isolating devices, and to otherwise disable machines or equipment to prevent unexpected energization, start up or release of stored energy in order to prevent injury to employees. At the time of the inspection, an employee received serious injuries when the screw auger he was working on was started unexpectedly. The screw auger had not been isolated from the hydraulic energy source of the tender truck. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citations.
