29 CFR 1910.132(a): Protective equipment was not used, when necessary, whenever hazards capable of causing injury and impairment were encountered: At the jobsite located at the intersection fo SE 50th and SE 70th in Chula, MO, on or about April 19, 2023: Employees were exposed to chemical burns while performing maintenance work on a tractor with toolbar attachment containing anhydrous ammonia. The employer did not ensure the appropriate personal protective equipment was worn before the employee performed the maintenance work. The employee received first and second-degree chemical burs to his left arm, stomach, and legs.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: At the jobsite located at the intersection fo SE 50th and SE 70th in Chula, MO, on or about April 19, 2023. Employees were exposed to chemical burns while performing maintenance work on a tractor with toolbar attachment containing anhydrous ammonia. The procedures in the employer's written LOTO program were not followed to ensure lines were depressurized before the employee performed maintenance work.
