29 CFR 1910.178(l)(2)(ii): Training shall consist of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace. The evaluation of a Raymond Stand up Forklft operator's training was inadequate in that the stand up forklift operator was not evaluated at the speed that reflected actual workplace conditions.

29 CFR 1910.178(n)(8): Under all travel conditions the truck shall be operated at a speed that will permit it to be brought to a stop in a safe manner. On October 6, 2016, a Raymond Stand up forklift operator lost control of the forklift by operating at a speed where he could not stop in time while backing up after picking up load and the forklift struck a storage rack.
