29 CFR  1904.40(a):The employer did not provide an authorized government representative the records within the four business hours:  Impact Fulfillment Services, LLC, Edwardsville, IL.  On and before October 1, 2021, the employer did not provide copies of injury and illness records for calendar year 2021 to an authorized representative within four business hours of the request.  Impact Fulfillment Services, LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1413131, citation number 2, item number 1 and was affirmed as a final order on October 22, 2019, with respect to a workplace located at 2035 Innis Road, Columbus, OH 43224.

29 CFR  1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent:   Impact Fulfillment Services, LLC, Edwardsville, IL.  On or about July 23, 2021, the employer did not record the following workplace injuries for temporary employees on the OSHA Form 300 or equivalent for the calendar year 2021:  A.  Upper Back Strain/Sprain due to lifting heavy rails to setup a line, which occurred on or about June 1, 2021, and resulted in days away from work.  B.  Lower Arm Strain/Sprain while lifting a box, which occurred on or about July 8, 2021, and resulted in a job transfer or restriction.  C.  Foot injury involving a forklift, which occurred on or about July 14, 2021, and resulted in a hospitalization and days away from work.  Note:  29 CFR 1904.31(a) and (b)(2):  You must record on the OSHA 300 Log the recordable injuries and illnesses that occur to employees who are not on your payroll if you supervise these employees on a day-to-day basis.

On or about July 23, 2021, the employer did not fill out an OSHA Form 301 or equivalent for each recordable injury:  A.
