29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652:  At the intersection of Tri-Park Way and North Systems Drive in Appleton Wi: On or about October 14, 2016 the employer had employees exposed to cave-in hazards while working in an excavation.   a.  Employees were exposed to cave-in hazards from the East wall where the excavation was approximately 14 feet deep  without any form of adequate shielding, shoring, sloping, or benching.   b.  Employees were exposed to cave-in hazards from the South wall where the excavation was approximately 15 feet deep and was lined with metal sheeting that was not adequately supported with wales and cross braces.
