Location: Employer's yard @ 555 South Street, Quincy, MA 02169 On or about October 18, 2016, employees were exposed to fall hazards and an employee fell more that 14 feet to the ground below, where fall protection was not used or provided.

29 CFR 1910.180(h)(3)(iii)(b): During the hoisting, care was not taken that the load did not contact any obstructions. Location: Employer's yard @ 555 South Street, Quincy, MA 02169 On or about October 18, 2016 employees were exposed to struck-by hazards while hoisting into areas, where the load came in contact with known obstructions and an employee sustained an amputation injury.
