29 CFR 1926.251(a)(1): Rigging equipment for material handling was not inspected prior to use on each shift and as necessary during its use to ensure that it was safe. (a) Worksite: On or about 9/30/2025, employees were exposed to struck by hazards when a used Firestone 65/35-33 L-5 tire, configured below the hook to lift and drop a chisel, was not proof tested prior to being put into use, nor was it marked to indicate their safe working capacity. The employer did not inspect the rigging as necessary during its use.
