29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide:  a)  At Lot 59, 1250 Falls Crest, Auburn, Alabama:  On or about October 18, 2016 and at times prior:  the employer allowed his employee to work from a job made scaffold located on a the second floor with a walk board that only measured 10 inches wide exposing his employee to a fall hazard of 25 feet to the floor below.

29 CFR 1926.304(d): Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe:  a)  At 1250 Falls Crest, Lot 59, Auburn, Alabama:  On or about October 18, 2016 and at times prior; the employer allowed his employee to operate a Makita Skill Saw on site which had a small piece of wood wedge into the guard exposing the blade and exposing his employees to an amputation hazard.    THE OMAR GARCIA DBA OMAR GARCIA WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.501(B)(13), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1103366 CITATION NUMBER 1, ITEM NUMBER 1, AND WAS AFFIRMED AS A FINAL ORDER ON DECEMBER 31, 2015, WITH RESPECT TO A WORKPLACE LOCATED AT 180 BARKLEY STREET, LOT 49, PRATTVILLE, ALABAMA.
