OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees.   a)  4208 Integrity Way, Powder Springs, Ga., On or about October 19/2016, in the interior front side of the residential structure, the employer exposed employees to fall hazards from the second level 10 feet 4 inches above lower, in that the job-made ladder was used to access to the second floor did not have parallel side rails running the entire length of the ladder, evenly spaced and perpendicular step treads and was not secured from displacement.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  (a) 4208 Integrity Way, Powder Springs, Ga., On the front side of a multi-family residential home, employees were not protected from eye injuries while installing shingles with a Pneumatic Hitachi Stick Frame Nail Gun without safety glasses.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) 4208 Integrity Way, Powder Springs, GA: An employee, was installing trusses on a mullti-family dwelling without fall protection, and employee was exposed to fall hazards in excess of 25 feet above the ground.  b) 4208 Integrity Way, Powder Springs, GA: Employees were framing inside walls of a mullti-family dwelling without fall protection, exposing employees to a fall to a lower level of approximately 10 feet 4 inches.
